Maple Knoll Home Health Services celebrates 15 years
of service.
In 1995, Bobbi Graham saw a need for the residents of Maple Knoll Village.
“The campus needed to provide short term skilled nursing care
to help them recover from illness and injuries,” says Graham,
now the Administrator of Home Health Services for Maple Knoll Communities.
“For the past 15 years our program has helped our residents manage
their diseases so that they can remain in their apartments and cottages.”
After several years of providing services on campus, Maple Knoll began
offering home health services to the community, expanding to most of
Butler and Hamilton Counties. This includes the newly opened office
at Bayley Place in Delhi.
The home health team consists of nurses, home health aides, physical,
occupational, and speech therapists. The staff works hand in hand with
clients, family and physicians to tailor a personalized plan for individuals.
This Medicare-certified home health agency is available to individuals
age 60 and older, recovering from an illness, injury or surgery and
requiring health care services. They accept many other health care plans
in addition to Medicare.
The Maple Knoll Home Health Service program is recognized as a leader
in preventing unplanned hospital stays for their clients. In 2005 Maple
Knoll Home Health Services was recognized as a National Best Practice
Agency. The agency is in the top five percent of agencies in Ohio and
in the top ten percent in the nation in preventing unplanned hospital
stays.
