The Geriatric Evaluation Center was created to provide special care for older adults and their families, whether it’s a complete medical evaluation or advice about a lifestyle question.

As people get older, staying healthy and active is more important than ever. We believe older adults should enjoy this time of life in the best health and as free from worry as possible. Older people and their families need reliable medical information to help them understand the changes that occur with aging. Accurate information, coupled with support and advice, can help older adults live fully and productively with whatever limitations they may experience as they grow older.

Now there is one source older adults and their families can rely on for medical advice and support. The Geriatric Evaluation Center is designed for people 65 and older. Conveniently located at Maple Knoll Village in Springdale, the center offers expert advice to manage health concerns and maintain the well-being of the older adult.

The Geriatric Evaluation Center helps people find solutions, whether it is sorting out an older adult’s complex medical problems, providing advice on coping with memory loss, or helping a family make a decision about living arrangements. With your written permission, the center will keep your personal physician informed of its findings and recommendations.

What to Expect

When you call The Geriatric Evaluation Center, we will take the time to talk with you about your specific concerns. We will send to you a confirmation letter, map, and medical record release form(s) so you may request any pertinent medical records be forwarded to our office in advance of the appoint-ment.

Our medical assessment includes a comprehensive physical examination and meetings with appropriate medical specialists. Because our assessments are so thorough, we allow two hours. Our team uses this time to put together a comprehensive medical picture, so we can make the best treatment recommendations.

A one-hour return follow-up appoint-ment is scheduled one week after the first assessment. This is an opportunity for you and your family to ask additional questions and learn about our recommendations.

Upon your signed release, all informa-tion will be shared fully with you and your family members as well as with your personal physician.

Telephone information service to identify and locate community services and other resources useful to older adults and their families

Guidance with long-term living arrangements, in-home or relocation alternatives

Each evaluation is tailored to the specific needs of the older adult. Our role is to determine what steps an older adult can reasona-bly take to make life easier and more satisfying. In every case, we try to find a solution that will allow a person to remain as independent as possible.
